Skilled Home Health Care and Home Infusion Therapy Scheduling Initiative

The Skilled Home Health Care/Home Infusion Therapy Scheduling Initiative is for Veterans Choice Program (VCP)- and Patient-Centered Community Care (PCCC) program-eligible veterans.

The veteran’s local U.S. Department of Veterans Affairs Medical Center (VAMC) assumes the responsibility of care coordination, including scheduling appointments, for veterans authorized to receive skilled home health care and/or home infusion therapy.

Provider Information Packets

Once home health care/home infusion sessions have been scheduled, the VAMC will issue an information packet with important appointment details to the provider and notify Health Net Federal Services, LLC (HNFS) of the appointment. HNFS will send an authorization letter to the veteran and provider with important information about the services authorized.

Requests for Additional Services

If additional services are required, outside of the initial authorization, requests will be reviewed and processed by the veteran’s VAMC.

Medical Documentation

For skilled home health care/home infusion therapy services, fax complete medical documentation to HNFS within the time frame indicated in the provider packet, with the following exception:

If the VAMC listed on the provider packet participates in a scheduling initiative, medical documentation must be returned to the VAMC, not HNFS. These directions will be included on the provider packet HNFS distributes to the initial servicing provider. To date, the following VAMCs participate in a scheduling initiative:

  • Fargo, North Dakota (PCCC/VCP)
  • Ft. Harrison, Montana (VCP only)
  • Madison, Wisconsin (VCP only)
  • Tomah, Wisconsin (VCP only)
