*If you know someone who would benefit from case/care management or chronic care/disease management, please make a referral by completing the online nomination or Medical Management Nomination form and mailing or faxing it to HNFS. A case, care or disease manager will contact the beneficiary and his or her physician to discuss the individual health care needs.
