Your center point will be derived based on your:

A. Good Results

City and state - If an address match does not exist and the ZIP Code is not found or entered, your center point will be the center of the city you enter.

B. Better Results

ZIP Code - If an address match does not exist, your center point will be the center of the ZIP Code you enter.

C. Best Results

Address - If you provide a matchable complete address, your center point will be the address.

If a search based on city does not generate the results you expect, use a ZIP Code for a more accurate search. You can look up a ZIP Code using the link "Find my ZIP Code" which will bring you to the US Post Office ZIP Code Lookup service.

States included in this list represent the states covered within the TRICARE North region by Health Net Federal Services.

This identifies Veterans Administration (VA) providers affiliated with the Department of Veterans Affairs.

Are you a veteran? This identifies non-institutional Health Net Federal Services TRICARE network providers who accept requests from the Department of Veterans Affairs (DVA) to provide care to veterans (VAs).

Identifying these providers allows the DVA to contact the provider directly to request care for VAs on a case-by-case basis. Although there is no obligation for the provider to see the VA patient, if services are provided, any documentation and reimbursement of the care rendered to the VA patient is managed between the VA Medical Center (VAMC) and the provider.

The referral and instructions for seeking reimbursement from the VAMC will be provided by the patient at the time of the appointment.

Are you a CHAMPVA beneficiary? This identifies Health Net Federal Services TRICARE network providers who agree to accept assignment for Civilian Health and Medical Program of the Veteran's Administration (CHAMPVA) claims.

These providers will see CHAMPVA beneficiaries as allowed by their office appointment scheduling practices, and will not allow preferential appointment scheduling to CHAMPVA over TRICARE appointments. Although these network providers are encouraged to meet TRICARE access standards for CHAMPVA beneficiaries, it is not required.

These providers are furnished with the CHAMPVA claims processing instructions for submitting CHAMPVA claims to the VA Health Administration Center for payment.

TRICARE Prime:
TRICARE Prime is the managed health care option for active duty services members, retired service members and their families. TRICARE Prime enrollees have a primary care manager (PCM) and receive their care from the military treatment facility (MTF) or the TRICARE network of civilian providers. TRICARE Prime is only available in select areas and may require prior-authorization and/or referrals for specialty care.

TRICARE Prime Remote (TPR):
TRICARE Prime Remote (TPR) provides health care coverage similar to TRICARE Prime for active duty service members and their families who are on remote assignment. Retired service members and their families are not eligible for TPR. Beneficiaries enrolled in TPR have a primary care manager (PCM) and may require prior-authorization and/or referrals for specialty care.

TRICARE Standard:
TRICARE Standard, formerly known as CHAMPUS, is a fee-for-service option available to all TRICARE-eligibles except active duty service members. Beneficiaries can see any TRICARE network or non-network provider and are responsible for fiscal year deductibles and cost-shares.

TRICARE Reserve Select (TRS):
TRICARE Reserve Select (TRS) is a premium-based plan available to qualified National Guard and Reserve members, family members and survivors, offering health care coverage similar to TRICARE Standard. Members can see any TRICARE network or non-network provider and are responsible for fiscal year deductibles and cost-shares.

TRICARE Retired Reserve (TRR):
TRICARE Retired Reserve (TRR) is a premium-based plan available to qualified Retired Reserve members, family members and survivors, offering health care coverage similar to TRICARE Standard. Members can see any TRICARE network or non-network provider and are responsible for fiscal year deductibles and cost-shares.

TRICARE Young Adult Prime:
TRICARE Young Adult (TYA) Prime is the premium-based managed health care option for young adults who have aged out of TRICARE and are under age 26. Beneficiaries enrolled in TYA Prime have a primary care manager (PCM) and receive their care from the military treatment facility (MTF) or the TRICARE network of civilian providers. TYA Prime is only available in select areas and may require prior-authorization and/or referrals for specialty care.

TRICARE Young Adult Prime Remote:
TRICARE Young Adult (TYA) Prime Remote is premium-based option for young adults who have aged out of TRICARE and are under age 26. TYA Prime Remote provides coverage similar to TYA Prime for young adults who reside with their active duty sponsor in a TRICARE Prime Remote area. Beneficiaries enrolled in TYA Prime Remote have a primary care manager (PCM) and may require prior-authorization and/or referrals for specialty care.

TRICARE Young Adult Standard:
TRICARE Young Adult (TYA) Standard is a premium-based option for young adults who have aged out of TRICARE and are under age 26. TYA Standard provides health care coverage similar to TRICARE Standard. Members can see any TRICARE network or non-network provider and are responsible for fiscal year deductibles and cost-shares.
