W-9 Requirement

Health Net Federal Services, LLC (HNFS) requires the submittal of a Request for Taxpayer Identification Number and Certification (W-9) form in order to issue claims payment for Veterans Choice Program and Patient-Centered Community Care claims.

Health Net Federal Services requires a W-9 as it:

  • Provides accurate billing information to expedite claims processing;
  • ensures HNFS has complete and accurate provider records for future authorizations and claims; and
  • allows for accurate 1099 reporting.
