Returning Medical Documentation

Timely return of medical documentation is necessary to ensure coordination of care with U.S. Department of Veterans Affairs (VA) providers.

Providers must fax medical documentation to 1-855-300-1705 within the time frame indicated in the provider packet. This provides Health Net Federal Services, LLC (HNFS) sufficient time to review and deliver medical documentation to VA, per contract requirement. Failure to submit medical documentation in a timely manner may result in recoupment efforts.

VA may request medical documentation that cannot be scanned or faxed, for example a CD of images. In these cases, the non-scannable medical documentation must be mailed to the VA requesting the information.

  • Veterans Choice Program: The VA and address information is located on the 10-0386 for wait list and choice first eligible veterans and can be found on www.va.gov for mileage eligible veterans.
  • Patient-Centered Community Care: The VA and address information is located on the 10-7079 for outpatient services and the 10-7078 for inpatient services.

Tips for returning medical documentation:

  • Deliver medical documentation to HNFS immediately following the first appointment or discharge. Report missed or canceled appointments to HNFS.
  • Refer to the Required Medical Documentation Content checklists to ensure all elements are complete.
  • Return medical documentation to HNFS, even if VA has also requested a copy.
  • Use the Veteran-specific cover sheet from your provider notification packet to expedite medical documentation processing. The cover sheet contains a unique bar code, which is specific to a single episode of care for the Veteran. Our generic fax cover sheet can be used when the Veteran-specific bar-coded fax cover sheet is not available. Note: Processing will be delayed if the cover sheet is copied/used for another episode of care or combined with documentation for multiple authorizations.
  • Do not submit claims with medical documentation, as we cannot accept faxed claims for processing.
