Below are toolkits and additional information related to cancer and diabetes care.

Cervical Cancer

The Cervical Cancer and HPV Project Provider Toolkit from the California Medical Association Foundation provides information and resources regarding cervical cancer and HPV. It is supported by the American Congress of Obstetricians and Gynecologists, Association of American Physicians, California Academy of Physician Assistants, California Association of Nurse Practitioners and California Association of Family Physicians, and is available in print and electronically.

Breast Cancer

The National Cancer Institute conducts and supports research, training and other programs on the cause, diagnosis, prevention and treatment of cancer, rehabilitation from cancer, and the continuing care of cancer patients and their families.

Colorectal Cancer

The American Cancer Society offers free materials to help you promote colorectal cancer screening among your patients.

Diabetes

The American Diabetes Association offers a variety of educational materials through its website and call-in center (800-DIABETES). The materials are generally free for downloading, provide information on a variety of topics, and are written at various literacy levels and in several languages. (Note: There is a small cost for some materials.)

The National Diabetes Educational Program offers a variety of patient education materials that can be downloaded and distributed. Because these are not copyrighted, clinics can add logos and contact information.
